The two units measure the same thing in different ways. mg/dL is a mass concentration, milligrams of glucose per decilitre of blood, and is standard in the United States, India and much of Asia. mmol/L is a molar concentration and is standard in the UK, Europe, Canada and Australia. Converting between them is a single division by 18.0182, which is glucose's molar mass divided by 10.
Interpretation depends entirely on when the sample was taken. Fasting thresholds assume at least eight hours without food; the two-hour thresholds come from the oral glucose tolerance test, where a measured glucose load is given and the response measured. A reading taken an hour after lunch belongs to neither scale, which is why this tool asks.
The estimated A1C shown alongside is a translation, not a prediction. It answers the question of what A1C would correspond to this value IF this were your average glucose over two to three months. A single reading almost never is, so treat that figure as a way of orienting yourself between two scales rather than as a result.
Meter accuracy is worth knowing about. Consumer glucose meters are typically permitted around 15 percent deviation from a laboratory reference, so a reading of 126 could plausibly represent anything from about 107 to 145. That range straddles a diagnostic threshold, which is exactly why diagnosis is made on laboratory tests and repeated measurements.
